Phoenix Hockey Club Waiver & Release of Liability

This Waiver and Release of Liability Agreement (“Agreement”) is entered into by the undersigned participant (and, if applicable, the participant’s parent or legal guardian) in favor of Phoenix Field Hockey Club, its directors, officers, coaches, employees, volunteers, agents, independent contractors, and affiliates (collectively, “Releasees”).

1. Acknowledgment of Risks & Assumption of Risk

I understand and acknowledge that participating in field hockey activities—including but not limited to games, practices, tournaments, clinics, and conditioning—involves inherent risks of serious bodily injury, illness, permanent disability, paralysis, or death. Risks include direct contact with field hockey balls, sticks, or other equipment; collisions with players, spectators, or obstacles; field surface hazards; environmental conditions; and physical exertion. I voluntarily assume full responsibility for all risks associated with participation, whether caused by the negligence of Releasees or otherwise.

2. Medical Fitness & Authorization

I certify that the participant is physically fit, in good health, and properly conditioned to safely participate in field hockey activities. In the event of an injury or emergency, I authorize Phoenix Field Hockey Club staff to secure necessary medical treatment, emergency transportation, and hospital care on my behalf, understanding that Phoenix Hockey Club does not provide any medical services and that I am solely responsible for all associated costs.

3. Release, Waiver, & Covenant Not to Sue

To the fullest extent permitted by law, I hereby release, waive, discharge, and promise not to sue Phoenix Field Hockey Club or the Releasees from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, illness, or injury (including death) sustained by the participant, whether caused by the active or passive negligence of the Releasees or otherwise while participating in or traveling to/from club activities.

4. Indemnification

I agree to indemnify, defend, and hold harmless the Releasees from any and all liabilities, losses, damages, legal costs, or expense (including reasonable attorney fees) incurred as a result of the participant’s involvement in club activities or any breach of this Agreement.

5. Media & Photo Release

I grant Phoenix Field Hockey Club permission to photograph, videotape, or capture audio of the participant during activities and to use such media for promotional, educational, or media purposes without financial compensation.

6. Transportation Disclaimer

I understand that transportation to and from Club activities may be provided by parents, volunteers, or other participants. The Club assumes no responsibility or liability for any injuries, damages, or incidents that may occur during such transportation.

7. Media Release

I grant permission to the Club to use the Participant’s photograph, image, or likeness in publications, promotional materials, films, videos, websites, or social media without consideration or compensation. I acknowledge that the Club retains ownership and all rights to such materials.

8. Data Sharing

I acknowledge and agree that certain personal details (including name, phone number, address, and email) may be shared internally by the Club in connection with team formation, scheduling, and related activities.

9. Governing Law & Severability

This agreement shall be governed by the laws of the State of Arizona. If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
