Mandatory Waiver Submission Instructions

ACTION REQUIRED BEFORE FIELD ACCESS

Every coach, player, and rostered participant must execute a physical copy of the Phoenix Hockey Club Waiver of Liability prior to entering the playing surface. No exceptions. As the registered Team Manager, you hold sole responsibility for the collection and hand-off of these documents for your entire roster.

MANDATORY SUBMISSION PROTOCOL

• Physical Copies Only: Digital copies or verbal confirmations will not be accepted under any circumstances.
• Single Packet Delivery: All completed, signed physical waivers for your team must be organized and turned in together as one complete bundle/folder.
• Turn-In Location: Submit your complete team waiver packet directly to the Tournament Scoring Table prior to your team’s first scheduled practice, warm-up or game.
• Individual Player Clearance: No individual player will be permitted onto the fields or permitted to participate in any hockey related activities until their signed waiver is received and verified at the scoring table.

PENALTY FOR NON-COMPLIANCE: TEAM DISQUALIFICATION

Failure to submit a complete team waiver packet prior to game start will result in immediate Team Disqualification and instant forfeiture/discontinuance of play in the tournament. There will be no refunds of any fees for disqualified teams.

There will be no exceptions, delays, or extensions granted at game time. Ensure your administrative duties are finalized well before your match starts.
